Post DIEP flap results are excellent. However, note the upper cleavage and upper poles of the breasts. Sharp transition zones are seen between flap and native tissue. This is the target of fat grafting – to smoothen the transition zone and provide a more natural look.
Note specifically the upper poles of the bilateral breasts, where contour irregularities, seen in the above pictures, are effaced by the addition of the patients own fat.
